      from Monday -Friday, 7:30 AM - 4:30 PM to schedule a registration appointment and to take documents to the school site that your student will be attending.

You will need to take the documents below to the assigned school site to complete your child’s registration:

• Child’s Birth Certificate. Other acceptable items: baptismal record, passport or immigration papers

• Proof of Residence with your name & address: current utility bill, rental payment receipt or property tax payment receipt

• Immunization Record 

• Home Language Survey
• Oral Health Assessment
• Physical Examination Report

Your child will not be fully registered or placed in a class until all the necessary paperwork is received by their new school.
