Interns Student Classroom Observers and Reedley College Personnel

Please print a copy and provide it to the Intern or Student Observer. The Director or Principal must sign the application before HR can begin the application process for approval. 

Items needed for clearance:

• Application with signatures
• Valid driver's license/ID
• Valid TB test or TB Risk Assessment Questionnaire signed by a physician
• Master of Social Work Interns; including a COC (Certificate of Clearance)

*Starting Monday, September 19, 2022, the district will no longer require proof of vaccination or test weekly for those who are unvaccinated.

Once the Inter/ Student Observer has completed the packet along with the items listed above, please have them drop the packet off at the District Office. 

Once clearance has been completed, all parties will be notified and a district email will be created. 

*Reedley College personnel must also complete these forms.
